Anyone working for a Licensed Home Provider must complete a background check through the Office of Early Childhood must complete five steps:
Step 1 – DCF Form
Complete and sign an“Authorization for Release of Information from DCF” form.
Step 2 – FBI Form
Read and sign all three sections of the “FBI Privacy Notices”

Step 3 – CCHRS Enroll
You will need to pre-enroll in the CT Criminal History Request System (CCHRS).
You will need to use one of the SERVICE CODES listed below to pre-enroll.
- After you pre-enroll, you will see your Applicant Tracking Number on the confirmation page, you will also receive an email.
- You will need your Applicant Tracking Number in order to make a fingerprinting appointment with 211 Child Care in Step 4.
Service codes F322-6F2D OEC Child Care Center Staff/Volunteer or Prospective Staff/Volunteer ** Please note: if you are applying for a position, or currently work, in a child care facility that is operated by a public school district, a city or town, or another organization that is exempt from child care licensing and receives Care 4 Kids funds, then this is the correct service code: C760-56C9. If you are not sure, please check with your employer to be sure you enter the correct service code. |
Step 4 – Schedule
Schedule your appointment using our scheduling page. Please note that you will need the Applicant Tracking Number from completing your CCHRS form in step 3 to schedule an appointment.

Step 5 – Get Fingerprints
Have your fingerprints collected by 211 Child Care at the appointment you scheduled.
Please note that OEC is paying this fee for fingerprints submitted digitally or postmarked by April 1, 2022, or until the allocated funds are exhausted, whichever happens first.
